RESUMEN

OBJECTIVE: To understand Primary Health Care nurses' role in treating Lower Urinary Tract Dysfunction. METHOD: Cross-sectional multi-methodological research, composed of quantitative and qualitative steps, independently and sequentially. Data collected remotely, through a questionnaire and focus group, analyzed using descriptive statistics and thematic analysis by Braun and Clarke, respectively. The project was approved under Opinion 22691119.0.0000.0030. RESULTS: A total of 145 nurses participated in the study in the quantitative step and 20 in the qualitative step, working in Primary Health Care in Brazil. Of the 93.1% nurses who reported having already cared for people with Urinary Tract Dysfunction, only 54.4% provided guidance, mainly for training the pelvic floor muscles. CONCLUSION: Even though they have legal support and access to demand, nurses do not have the knowledge to offer conservative treatment for Lower Urinary Tract Dysfunction. Despite this, they were motivated to do so as long as they received specific training.

RESUMEN Objetivo: Comprender el papel del enfermero de Atención Primaria de Salud en el tratamiento de la Disfunción del Tracto Urinario Inferior. Método: Investigación transversal multimetodológica, compuesta por etapas cuantitativas y cualitativas, de forma independiente y secuencial. Datos recopilados de forma remota, a través de un cuestionario y un grupo focal, analizados mediante estadística descriptiva y análisis temático de Braun y Clarke, respectivamente. El proyecto fue aprobado bajo Opinión 22691119.0.0000.0030. Resultados: Participaron del estudio 145 enfermeros en la etapa cuantitativa y 20 en la cualitativa, actuando en la Atención Primaria de Salud en Brasil. Del 93,1% de los enfermeros que afirmaron haber atendido ya a personas con Disfunción del Tracto Urinario, sólo el 54,4% brindó orientación, principalmente para el entrenamiento de los músculos del suelo pélvico. Conclusión: Incluso con apoyo legal y acceso a la demanda, los enfermeros no tienen el conocimiento para ofrecer tratamiento conservador para la Disfunción del Tracto Urinario Inferior. Pese a ello, estaban motivados para hacerlo siempre que recibieran una formación específica.


RESUMO Objetivo: Compreender a atuação dos enfermeiros da Atenção Primaria à Saúde no tratamento da Disfunção do Trato Urinário Inferior. Método: Pesquisa transversal multi-metodológica, composta por etapas quantitativa e qualitativa, de forma independente e sequencial. Dados coletados de forma remota, por meio de questionário e grupo focal, analisados por estatística descritiva e análise temática de Braun e Clarke, respectivamente. O projeto foi aprovado sob Parecer no. 22691119.0.0000.0030. Resultados: Participaram do estudo 145 enfermeiros na etapa quantitativa e 20 na qualitativa, atuantes na Atenção Primária à Saúde do Brasil. Dos 93,1% enfermeiros que referiram já terem atendido pessoas com Disfunção do Trato Urinário, apenas 54,4% prestaram orientações, sendo principalmente para treinamento da musculatura do assoalho pélvico. Conclusão: Mesmo possuindo respaldo legal e acesso à demanda, os enfermeiros não têm conhecimento para oferecer tratamento conservador para Disfunção do Trato Urinário Inferior. Apesar disso, mostraram-se motivados para tal atuação desde que recebam capacitação específica.

BACKGROUND: Germ cell tumors (GCTs) comprise a rare and heterogeneous group of neoplasms presenting different clinical and histological characteristics, leading to a challenging scenario in clinical practice. Diffusion-weighted imaging (DWI) has been suggested as an indirect marker of tumor density and cellularity and could be used to monitor therapeutic response. However, its role in pediatric GCTs needs to be clarified. PURPOSE: Here, we evaluated the features of DWI in pediatric extracranial GCTs in a reference Brazilian institution. MATERIAL AND METHODS: We included 43 pediatric patients with primary GCTs treated between 2008 and 2022 in Hospital de Amor de Barretos. The patients' MRI images included T1-weighted without contrast, T2-weighted, DWI and apparent diffusion coefficient (ADC) maps. DWI was evaluated in the section that exhibited the greatest restricted diffusion in the largest hypersignal area of the image. The lowest ADC value was determined to define the region of interest (ROI). We used a small ROI, avoiding necrotic, adipose tissue, noisy or nonenhancing lesion voxels as recommended. ROI determination was established by visual inspection by two radiologists in accordance. We used two values of b (b = 50 mm2/s or b = 800) for ADC values. RESULTS: The highest mean ADC (mADC) value was observed in pure teratomas (1,403.50 ± 161.76 x10-3 mm2/s; mean ± SD) compared to other histologies (yolk sac, mixed teratoma, dysgerminoma and mixed GCT) of GCT (p<0.001). Furthermore, ROC analysis determined a cutoff mADC value of 1,179.00 x 10-3 mm2/s that differentiated pure teratomas from the other GCT histologies with a sensitivity of 95.8% and a specificity of 92.9% (AUC = 0.979; p<0.01). A significant increase in mADC was observed for malignant GCTs in treatment (1,197.00 ± 372.00 mm2/s; p<0.001) compared to that exhibited at the time of diagnosis (780.00 ± 168.00 mm2/s; mean ± SD. Our findings suggest that mADC assessment could be used as a tool to distinguish pure teratomas from malignant CGT histologies at diagnosis. Additionally, we demonstrated reasonable evidence that it could be used as a complementary tool to monitor treatment response in patients with malignant GCT.

PURPOSE: We mapped key concepts and identified 4 fundamental nursing interventions for patients with neurogenic bowel dysfunction due to complete spinal cord injury (SCI). METHODS: A scoping review was conducted according to the recommendations of the Joanna Briggs Institute. SEARCH STRATEGY: Searches were performed in PubMed, LILACS, CINAHL, COCHRANE, and SCOPUS electronic databases. We use searched the gray literature using the Google Scholar search engine. We formulated a question to guide the search, based on the participants, concept, and context format: "What are the key manual nursing interventions performed in patients with neurogenic bowel dysfunction resulting from complete spinal cord injury?" We included nursing intervention strategies that may be performed by health professionals, patients, or caregivers. Two reviewers independently participated in the selection; disagreements were resolved by a third reviewer and 5 experts. FINDINGS: Thirteen studies conducted between 1998 and 2019 were selected; 5 were randomized clinical trials. Four main interventions were identified for conservative management of neurogenic bowel dysfunction in patients with complete SCI. They were digital-anal stimulation, manual extraction of feces, abdominal massage, and strategies used to stimulate the gastrocolic reflex. CONCLUSIONS: Research suggests that each of these interventions, administered alone or in combination, supports bowel evacuation in patients with a complete SCI. Each of these interventions may be performed by a nurse, and taught to the patient and/or lay caregiver. IMPLICATIONS FOR PRACTICE: An individualized bowel management program for patients with neurogenic bowel dysfunction due to SCI is necessary to ensure regular bowel evacuation, preserve fecal continence, and support dignity and health-related quality of life. The conservative interventions identified in this scoping review should be incorporated in protocols or guidelines for management of neurogenic bowel dysfunction in this vulnerable population.

RESUMEN

Ovarian germ cell tumors (OGCTs) are rare in adults; indeed, they occur predominantly in children, adolescents, and young adults, and they account for approximately 11% of cancer diagnoses in these groups. Because OGCTs are rare tumors, our current understanding of them is sparse; this is because few studies have investigated the molecular basis of pediatric and adult cancers. Here, we review the etiopathogenesis of OGCTs in children and adults, and we address the molecular landscape of these tumors, including integrated genomic analysis, microRNAs, DNA methylation, the molecular implications of treatment resistance, and the development of in vitro and in vivo models. An elucidation of potential molecular alterations may provide a novel field for understanding the pathogenesis, tumorigenesis, diagnostic markers, and genetic peculiarity of the rarity and complexity of OGCTs.

RESUMEN

OBJECTIVE: To report the experience of creating an online extension course, focusing on Advanced Nursing Practice in the promotion ofchild continence. METHOD: An experience report on the creation of a course for nursing students, carried out in the second half of 2021 at a federal university in Brazil. It was based on the Meaningful Learning Theory, Instructional Design, and on the Digital Storytelling strategy. RESULTS: The course was planned to be offered online, including knowledge about childhood continence, Advanced Nursing Practice, urinary and intestinal symptoms, and nurses' work in the context of pediatric urology. FINAL CONSIDERATIONS: The authors proposed an innovative online course based on their experience to promote the teaching of child urological care in nursing education.

RESUMO Objetivo: descrever vivências de mães e repercussões da internação de crianças crônicas dependentes de tecnologias em Unidade de Terapia Intensiva Pediátrica na vida pessoal e familiar. Método: estudo qualitativo realizado com 11 mães de crianças crônicas dependentes de tecnologias de um hospital público do Distrito Federal, Brasil. Os dados foram coletados por entrevistas semiestruturadas, gravadas, posteriormente, transcritas, codificadas e subdivididas em categorias. Utilizou-se análise de conteúdo temática como estratégia de análise. Resultados: foram divididos em cinco categorias, que expressam as experiências da hospitalização prolongada, sentimentos maternos, dificuldades/necessidades, estratégias de enfrentamento e contexto da pandemia, que destacaram as repercussões majoritariamente negativas que trazem mudanças na dinâmica familiar de modo geral, ao precisarem se reinventar e se adaptar às condições da criança e do hospital. Considerações finais: os dados evidenciam a vulnerabilidade familiar, e reforçam a importância da atuação da enfermagem para adoção de um modelo de cuidado centrado na família.


ABSTRACT Objective: to describe mothers' experiences and repercussions of the hospitalization of technology-dependent chronic children in a Pediatric Intensive Care Unit on their personal and family life. Method: qualitative study conducted with 11 mothers of technology-dependent chronic children in a public hospital in the Federal District, Brazil. Data were collected by semi-structured interviews, recorded, later transcribed, coded, and subdivided into categories. Thematic content analysis was used as an analysis strategy. Results: were divided into five categories, which express the experiences of prolonged hospitalization, maternal feelings, difficulties/needs, coping strategies, and context of the pandemic, which highlighted the mostly negative repercussions that bring changes in the family dynamics in general, by needing to reinvent themselves and adapt to the conditions of the child and the hospital. Final considerations: the data show family vulnerability and reinforce the importance of nursing work for the adoption of a family-centered care model.


RESUMEN Objetivo: describir las experiencias de las madres y las repercusiones de la hospitalización de niños crónicos dependientes de tecnología en una Unidad de Cuidados Intensivos Pediátricos en su vida personal y familiar. Método: estudio cualitativo realizado con 11 madres de niños dependientes crónicos de tecnología de un hospital público del Distrito Federal, Brasil. Los datos se recogieron mediante entrevistas semiestructuradas, grabadas, posteriormente transcritas, codificadas y subdivididas en categorías. Se utilizó el análisis de contenido temático como estrategia de análisis. Resultados: se dividieron en cinco categorías, que expresan las experiencias de hospitalización prolongada, los sentimientos maternos, las dificultades/necesidades, las estrategias de afrontamiento y el contexto de la pandemia, que pusieron de relieve las repercusiones, en su mayoría negativas, que provocan cambios en la dinámica familiar en general, ya que tienen que reinventarse y adaptarse a las condiciones del niño y del hospital. Consideraciones finales: los datos muestran la vulnerabilidad familiar y refuerzan la importancia de la actuación enfermera para la adopción de un modelo de atención centrado en la familia.

OBJECTIVE: To map available scientific evidence about the pediatric population with spina bifida submitted to transanal irrigation to manage signs and symptoms of neurogenic bowel. SOURCE OF DATA: This research was developed according to recommendations from the Joanna Briggs Institute Reviewers' Manual and the PRISMA Extension for Scoping Reviews. Searches were carried out in the databases: CINAHL, Medline/Pubmed, Scielo, Scopus, Web of Science, Embase, LILACS, Proquest, and the CAPES catalog of theses and dissertations. Quantitative and qualitative studies on the topic were included, as long as they dealt with this population. There was no predetermined time frame. SUMMARY OF THE FINDINGS: The authors found 1.020 studies, selected 130 for close reading, and included 23 in the review, all of which had been published from 1989 to 2021. The authors mapped the characteristics of the studies, including their definitions of concepts and use of scales, criteria for the indication of transanal irrigation, training to carry out the procedure, devices and solutions used, number and frequency of transanal irrigations, health care actions, time spent, associated complications, complementary exams, adherence rate, follow-up, and outcomes, focusing on the benefits for bowel management. CONCLUSIONS: Despite the variability of evaluation parameters and term definitions, evidence suggests that transanal irrigation is a safe and effective method to manage fecal incontinence. Studies in the field are likely to grow, using standardized scales and longitudinal follow-ups. The authors suggest further research on transanal irrigation in the pediatric population with spina bifida in the Latin American context.

Consideraciones finales: Se propuse un curso innovador basado en la experiencia de los autores, con el objetivo de promover la enseñanza del cuidado urológico infantil en la formación de enfermeros.

RESUMO Objetivo mapear as evidências sobre as tecnologias baseadas na internet e sua aplicabilidade no cuidado ambulatorial e domiciliar em urologia pediátrica. Métodos trata-se de revisão de escopo, com buscas conduzidas em seis bases de dados, sem limite de tempo, nos idiomas português, inglês e espanhol. Resultados 2.200 artigos foram obtidos, sendo 19 publicações elegíveis para a amostra final. As tecnologias baseadas na internet mais utilizadas foram telessaúde e telemedicina (47,3%), programas de intervenção online (21,0%) e aplicativos móveis (15,8%), com uma taxa menor de uso de serious game s, mídias sociais e vídeos de micção (5,3%), majoritariamente no contexto domiciliar. Conclusão o mapeamento das evidências tecnológicas na assistência ambulatorial e domiciliar em urologia pediátrica permitiu identificar os tipos de tecnologia e suas aplicações voltadas para o autogerenciamento dos sintomas e autoeficácia, para o monitoramento e acompanhamento do cuidado urológico. Contribuições para a prática: as evidências sintetizadas contribuem para uma prática profissional expandida, qualificada e inovadora junto à população pediátrica com sintomas urinários e intestinais, características relacionadas a uma prática avançada de enfermagem, além de favorecer diagnóstico precoce e maior adesão terapêutica, particularmente por apoiar intervenções personalizadas às necessidades do paciente pediátrico e sua família.


ABSTRACT Objective to map the evidence on web-based technologies and their applicability in outpatient and home care in pediatric urology. Methods this is a scoping review, with searches conducted in six databases, with no time limit, in Portuguese, English, and Spanish. Results 2,200 articles were obtained, with 19 publications eligible for the final sample. The most used web-based technologies were telehealth and telemedicine (47.3%), online intervention programs (21.0%), and mobile apps (15.8%), with a lower rate of use of serious games, social media, and urination videos (5.3%), mostly in the home setting. Conclusion the mapping of technological evidence in outpatient and home care in pediatric urology allowed us to identify the types of technology and their applications focused on self-management of symptoms and self-efficacy, for monitoring and follow-up of urologic care. Contributions to practice: the synthesized evidence contributes to an expanded, qualified, and innovative professional practice with the pediatric population with urinary and bowel symptoms, characteristics related to advanced nursing practice, in addition to favoring early diagnosis and greater therapeutic adherence, particularly by supporting interventions tailored to the needs of the pediatric patient and his family.

Resumo Objetivo Descrever o perfil sociodemográfico, o acesso e interesse em receber informações on-line sobre a disfunção vesical e intestinal, bem como compreender a vivência da família de crianças e adolescentes acometidos por essa disfunção. Métodos Trata-se de estudo multi-metodológico realizado em um ambulatório de Prática Avançada de Enfermagem em Uropediatria de um hospital de ensino da região centro-oeste do país. Resultados A vivência da família da criança com disfunção vesical e intestinal aponta para um impacto negativo no cotidiano podendo estar relacionado às condições sociodemográficas, a falta de conhecimento das famílias sobre os sintomas e aos estigmas associados à sua manifestação. Para tanto, o acesso a informações on-line apresenta-se como potencial ferramenta de apoio para melhorar a experiência da família da criança com os sintomas. Conclusão Os resultados expressam a caracterização sociodemográfica da criança e sua família e o acesso e interesse em receber informações pela internet sobre a disfunção vesical e intestinal, que podem ser importantes para a adesão e percepção de melhora nos sintomas refletindo na vivência familiar. Portanto, o enfermeiro que atua no contexto de cuidado em uropediatria precisa inovar em sua abordagem e implementar novas modalidades de assistência ou intervenções em saúde, principalmente por meio da incorporação de tecnologias baseadas na internet, visando à melhora da qualidade de vida tanto da família quanto da criança com disfunção vesical e intestinal.


Resumen Objetivo Describir el perfil sociodemográfico, el acceso y el interés en recibir información digital sobre la disfunción vésico-intestinal, así como comprender la vivencia de la familia de niños y adolescentes acometidos por esta disfunción. Métodos Se trata de un estudio multimetodológico realizado en consultorios externos de Práctica Avanzada de Enfermería en Urología Pediátrica de un hospital universitario de la región Centro-Oeste del país. Resultados La vivencia de la familia de niños con disfunción vésico-intestinal indica un impacto negativo en la cotidianidad, lo que puede estar relacionado con las condiciones sociodemográficas, la falta de conocimiento de las familias sobre los síntomas y los estigmas asociados a su manifestación. Para eso, el acceso a la información digital se presenta como una potencial herramienta de apoyo para mejorar la experiencia de la familia de niños con los síntomas. Conclusión Los resultados expresan la caracterización sociodemográfica de los niños y su familia y el acceso e interés en recibir información por internet sobre la disfunción vésico-intestinal, que puede ser importante para la adhesión y percepción de mejora de los síntomas y puede reflejarse en la vivencia familiar. Por lo tanto, los enfermeros que actúan en el contexto de cuidado en urología pediátrica necesitan innovar su enfoque e implementar nuevas modalidades de atención o intervenciones en salud, principalmente mediante la incorporación de tecnologías basadas en internet, con el objetivo de mejorar la calidad de vida tanto de la familia como de los niños con disfunción vésico-intestinal.


Abstract Objective To describe the sociodemographic profile, access and interest in receiving online information about bladder and bowel dysfunction as well as understand the experience of families of children and adolescents affected by this dysfunction. Methods This is a multi-methodological study carried out in an outpatient clinic of Advanced Nursing Practice in uropediatrics of a teaching hospital in midwestern Brazil. Results The experience of families of children with bladder and bowel dysfunction points to a negative impact on everyday life, which may be related to sociodemographic conditions, lack of knowledge of families about the symptoms and stigmas associated with its manifestation. To this end, access to online information is a potential support tool to improve the experience of children's families with the symptoms. Conclusion The results express children's sociodemographic characterization and their family and the access and interest in receiving information on the internet about bladder and bowel dysfunction, which may be important for compliance and perception of improvement in symptoms, reflecting on family experience. Therefore, nurses who work in the context of uropediatrics care need to innovate in their approach and implement new care modalities or health interventions, mainly through the incorporation of internet-based technologies, aimed at improving the quality of life of both the families and children with bladder and bowel dysfunction.

Abstract Objective To map available scientific evidence about the pediatric population with spina bifida submitted to transanal irrigation to manage signs and symptoms of neurogenic bowel. Source of data This research was developed according to recommendations from the Joanna Briggs Institute Reviewers' Manual and the PRISMA Extension for Scoping Reviews. Searches were carried out in the databases: CINAHL, Medline/Pubmed, Scielo, Scopus, Web of Science, Embase, LILACS, Proquest, and the CAPES catalog of theses and dissertations. Quantitative and qualitative studies on the topic were included, as long as they dealt with this population. There was no predetermined time frame. Summary of the findings The authors found 1.020 studies, selected 130 for close reading, and included 23 in the review, all of which had been published from 1989 to 2021. The authors mapped the characteristics of the studies, including their definitions of concepts and use of scales, criteria for the indication of transanal irrigation, training to carry out the procedure, devices and solutions used, number and frequency of transanal irrigations, health care actions, time spent, associated complications, complementary exams, adherence rate, follow-up, and outcomes, focusing on the benefits for bowel management. Conclusions Despite the variability of evaluation parameters and term definitions, evidence suggests that transanal irrigation is a safe and effective method to manage fecal incontinence. Studies in the field are likely to grow, using standardized scales and longitudinal follow-ups. The authors suggest further research on transanal irrigation in the pediatric population with spina bifida in the Latin American context.

The association between chronic kidney disease (CKD) and pulmonary pathophysiological changes is well stablished. Nevertheless, the effects of aerobic exercise (AE) on lungs of CKD need further clarification. Thus, Swiss mice were divided in control, AE, CKD, and CKD + AE groups. CKD was induced by 0.2% adenine intake during 8 weeks (4 weeks of CKD induction and 4 weeks of AE). AE consisted in running on treadmill, at moderate intensity, 30 min/day, 5 days/week, during 4 weeks. Twenty-four hours after the last training day, functional capacity test was performed, and 48 h after the test, mice were euthanized. CKD mice showed a significant increase in urine output, serum urea, and creatinine concentrations, and decreased body weight and urine density, besides oxidative damage (p = 0.044), edema area (p < 0.001), leukocyte infiltration (p = 0.040), and collagen area in lung tissue (p = 0.004). AE resulted in an increase of distance traveled (p = 0.049) and maximum speed (p = 0.046), increased activity of catalase (p = 0.031) and glutathione peroxidase (p = 0.048) in lungs, increased levels of nitric oxide (NOx) in serum (p = 0.001) and bronchoalveolar lavage fluid (p = 0.047), and decreased kidney histological injury (p = 0.018) of CKD mice. However, AE also increased oxidative damage (p = 0.003) and did not change collagen content or perivascular edema in lungs (p > 0.05) of CKD mice. Therefore, AE attenuated kidney injury and improved antioxidants defenses in lungs. Despite no significant changes in pulmonary damage, AE significantly improved physical performance in CKD mice.

RESUMEN

OBJETIVO: Presentar un protocolo para ensayo clínico controlado y aleatorizado que estudie los resultados de la asociación entre técnicas de uroterapia estándar y mindfulness en la adhesión al programa y reducción de síntomas de disfunción vesical e intestinal en escolares. MÉTODO: Presentación descriptiva del protocolo. RESULTADOS: El grupo de control deberá recibir orientaciones de medidas comportamentales gradualmente en visitas semanales, durante cuatro semanas. Tales medidas deberán contemplar: control de la ingesta de agua, intervalo de evacuación, ingestión de potenciales irritantes de la vejiga, y entrenamiento muscular del fondo pélvico. El grupo experimental deberá ser sometido al mismo protocolo, además de las técnicas de atención plena (mindfulness), previamente al inicio de cada consulta de uroterapia. CONCLUSIÓN: Se espera estimular la aplicación de este protocolo en diferentes escenarios y así evaluar la contribución de la práctica de atención plena en la adhesión al tratamiento y en la reducción de síntomas.

RESUMO Objetivo: descrever as características clínico-epidemiológicas de crianças em idade escolar com disfunção vesical e intestinal atendidas em um ambulatório de enfermagem especializado. Método: estudo retrospectivo de abordagem quantitativa, descritivo e documental, realizado em prontuários de crianças atendidas em serviço de prática avançada de enfermagem em uropediatria, localizado em hospital universitário do Distrito Federal - Brasil, no período de abril a maio de 2019. Os dados foram analisados descritivamente e com teste qui-quadrado de Pearson para associações. Resultados: Todas as crianças apresentaram constipação intestinal funcional associada a sintomas urinários, sendo que os prevalentes foram urgência miccional (80%) e manobras de contenção (70%). Foi identificada significância estatística entre sexo e sintomas de urgência miccional, aumento e diminuição da frequência urinária. A Infecção do Trato Urinário foi a principal comorbidade (33%). Conclusão: esse estudo contribui para o planejamento e implementação de intervenções mais sensíveis e específicas do processo de cuidar em Uropediatria.


ABSTRACT Objective: to describe the clinical-epidemiological characteristics of schoolchildren with bladder and bowel dysfunction treated in a specialized Nursing outpatient service. Method: a retrospective study with a quantitative, descriptive and documentary approach, conducted from April to May 2019 with medical records of children treated in an advanced Nursing practice service specialized in Uropediatrics and located in a university hospital from Distrito Federal, Brazil. The data were analyzed both in a descriptive manner and through Pearson's chi-square test for associations. Results: All the children presented functional intestinal constipation associated with urinary symptoms, with prevalence of voiding urgency (80%) and retention maneuvers (70%). A statistically significant difference was identified between gender and voiding urgency symptoms, as well as with increased and reduced urinary frequency. The main comorbidity was Urinary Tract Infection (33%). Conclusion: this study contributed to planning and implementing more sensitive and specific interventions regarding the care process in Uropediatrics.

Método: estudio retrospectivo de enfoque cuantitativo, descriptivo y documental, llevado a cabo durante abril y mayo de 2019 con historias clínicas de niños atendidos en un servicio de práctica avanzada de Enfermería especializado en Uropediatría situado en un hospital universitario del Distrito Federal, Brasil. Los datos se analizaron en forma descriptiva y por medio de la prueba chi-cuadrado de Pearson para asociaciones. Resultados: todos los niños presentaron constipación intestinal funcional asociada con síntomas urinarios, con prevalencia de urgencia urinaria (80%) y maniobras de retención (70%). Se identificó significancia estadística entre el sexo de los participantes y los síntomas de urgencia urinaria, así como con aumento y disminución de la frecuencia. La comorbilidad más importante fue la Infección del Tracto Urinario (33%). Conclusión: este estudio generó aportes para planificar e implementar intervenciones más sensibles y específicas al proceso de atención uropediátrica.

Objetivo:levantar a ocorrência de sintomas urinários e intestinais em crianças da rede pública de ensino fundamental da capital paranaense. Método: estudo quantitativo, exploratório-descritivo e de corte transversal. Amostra de pais/responsáveis que preencheram os seguintes instrumentos: Dysfunctional Voiding Scoring System; critérios de Roma IV; escala de Bristol. Análise realizada pelo IBM SPSS Statistics v.20.0. Os dados foram coletados no período de agosto a outubro de 2019. Os critérios de inclusão eram a criança estar matriculada e frequentando regularmente o ensino fundamental e a entrega do questionário preenchido ou parcialmente preenchido. Resultados: Foram entregues 458 questionários. Dos totalmente preenchidos, 51,9% era de meninas, e a de idade foi de 7,7 anos; 83% das crianças apresentaram algum sintoma urinário e/ou intestinal de baixa gravidade. Sem diferença significativa de sintomas entre sexos, e com acréscimo significativo de gravidade em crianças com 7 anos ou menos. Sintomas mais prevalentes: frequência miccional reduzida, frequência evacuatória reduzida, esforço evacuatório, urgência miccional e manobras de contenção; 39,6% das crianças apresentavam Constipação Intestinal Funcional. Disfunção vesical e intestinal em 35 crianças, a maior prevalência no sexo feminino. Conclusão: alta ocorrência de sintomas urinários e intestinais nas crianças. O sintoma mais prevalente foi constipação.


Objective:survey the occurrence of urinary and intestinal symptoms in children from public elementary schools in the capital of Paraná. Method: quantitative, exploratory-descriptive and cross-sectional study. Sample of parents/guardians who completed the following instruments: Dysfunctional Voiding Scoring System; Rome IV criteria; Bristol scale. Analysis performed by IBM SPSS Statistics v.20.0. Data were collected from August to October 2019. Inclusion criteria were the child being enrolled and regularly attending elementary school and the delivery of the completed or partially completed questionnaire. Results: 458 questionnaires were delivered. Of those fully completed, 51.9% were girls, and the age was 7.7 years; 83% of the children had some urinary and/or intestinal symptoms of low severity. There was no significant difference in symptoms between genders, and with a significant increase in severity in children aged 7 years and under. Most prevalent symptoms: reduced voiding frequency, reduced defecation frequency, defecation effort, voiding urgency and containment maneuvers; 39.6% of the children had Functional Intestinal Constipation. Bladder and bowel dysfunction in 35 children, the highest prevalence in females. Conclusion: high occurrence of urinary and intestinal symptoms in children. The most prevalent symptom was constipation.

Síntomas más prevalentes: frecuencia miccional reducida, frecuencia evacuatoria reducida, esfuerzo evacuatorio, urgencia miccional y maniobras de contención; 39,6 % de los niños presentaban Constipación Intestinal Funcional. Disfunción vesical e intestinal en 35 niños, la mayor prevalencia en el sexo femenino. Conclusión: alta ocurrencia de síntomas urinarios e intestinales en niños. El síntoma más prevalente fue el estreñimiento.

OBJECTIVE: To present a proposal of a protocol for pelvic floor muscle evaluation and training to provide care to women with urinary incontinence. METHOD: Professional experience report, conducted in an outpatient facility for voiding dysfunctions in the Brazilian Unified Health System, conducted by an enterostomal therapist nurse. RESULTS: Elaboration of a proposal of care protocol in nursing consultations based on a directed physical examination, nursing diagnoses of the International Classification for Nursing Practice, and the adaptation of a muscle training protocol as prescription. Steps: static observation, dynamic observation, and palpation to verify the tension and evaluation of strength, sustentation, relaxation, and muscle coordination, followed by prescriptions for relaxation, proprioception, training for strength, sustentation, abdominal and pelvic coordination, and maintenance, as per diagnosis. CONCLUSION: This protocol proposal intends to support the evaluation of the pelvic muscles of women with urinary incontinence or at risk for developing this condition by nurses of all levels of healthcare, especially as part of nursing consultations in primary care.

BACKGROUND AND OBJECTIVE: Sepsis is a potentially deadly organic dysfunction, and one of the main causes of mortality in intensive care units (ICU). Aerobic exercise (AE) is a preventive intervention in the establishment of inflammatory conditions, such as chronic lung diseases, but its effects on sepsis remain unclear. Therefore, this study aimed to evaluate the effects of AE on health condition, mortality, inflammation, and oxidative damage in an experimental model of pneumosepsis induced by Klebsiella pneumoniae (K.p). METHODS: Animals were randomly allocated to Control; Exercise (EXE); Pneumosepsis (PS) or Exercise + Pneumosepsis (EPS) groups. Exercised animals were submitted to treadmill exercise for 2 weeks, 30 min/day, prior to pneumosepsis induced by K.p tracheal instillation. RESULTS: PS produced a striking decrease in the health condition leading to massive death (85%). AE protected mice, as evidenced by better clinical scores and increased survival (70%). AE alleviated sickness behavior in EPS mice as evaluated in the open field test, and inflammation (nitrite + nitrate, TNF-α and IL-1ß levels) in broncoalveolar fluid. Catalase activity, oxidative damage to proteins and DNA was increased by sepsis and prevented by exercise. CONCLUSION: Overall, the beneficial effects of exercise in septic animals encompassed a markedly improved clinical score and decreased mortality, along with lower inflammation markers, less DNA and protein damage, as well as preserved antioxidant enzyme activity. Neural network risk analysis revealed exercise had a considerable effect on the overall health condition of septic mice.

Objetivo: identificar as evidências científicas sobre a atuação de enfermagem na qualidade de vida de crianças e adultos em uso do cateterismo urinário e seus cuidadores no contexto de reabilitação. Método: revisão integrativa de literatura, através de questão norteadora "Qual a produção de conhecimento sobre a atuação de enfermagem na Qualidade de Vida de crianças e adultos em uso do cateterismo urinário e seus cuidadores no contexto de reabilitação?", em três bases de dados com os descritores cateterismo urinário, qualidade de vida e enfermagem. Foram incluídos 23 estudos, publicados no período de 2011 a 2020, realizados em mais de um país, em especial na população brasileira. Resultados: as principais atuações da enfermagem foram: educação em saúde, atividades de orientação em grupo, uso de simuladores e teleatendimentos. Conclusão: as contribuições do enfermeiro em reabilitação estão direcionadas ao cuidado integral e eficaz para promoção de melhores terapêuticas e uma maior qualidade de vida.


Objective: to identify the scientific evidence on nursing performance in the quality of life of children and adults undergoing urinary catheterization and their caregivers in the context of rehabilitation. Methods: integrative literature review adopting the guiding question "What is the production of knowledge about the nursing performance in the quality of life of children and adults undergoing urinary catheterization and their caregivers in the context of rehabilitation?" performed in three databases with use of the following descriptors: urinary catheterization, quality of life and nursing. Twenty-three studies published between 2011 and 2020, conducted in more than one country, especially in the Brazilian population, were included. Results: the main actions of nursing were: health education, group orientation activities, use of simulators and telenursing. Conclusion: the contributions of rehabilitation nurses are directed to comprehensive and effective care to promote better therapies and a higher quality of life.
